The QOC is a committee of volunteers made up of
   persons served, guardians, family members, interested
   citizens, & provider representatives who monitor:

      ▪  Services that are paid for are delivered, and services
         delivered are paid for
 
      ▪  Services are provided according to state regulation
         requirements
 
      ▪  Person served has basic health and safety needs
         met and has opportunities to make informed choices
         about where he/she would like to live and work
 
      ▪  Person served has the right to be treated with
         respect & is free of abuse/neglect
