Case Management services are those that assist the individual in gaining access to medical, social, educational,
and other needed services.  A Targeted Case Manager's (TCM) role includes any or all of the following services: 

• Assessment of an eligible individual to determine service needs
• Development of a Person-Centered Support Plan - More Info
• Referral and related activities
• Monitoring and follow-up activities to ensure care plan is implemented
• Transition and portability, including the planning of and arranging for services
