HCBS DD Waiver

The HCBS DD waiver (Home and Community Based Services Medicaid Waiver for individuals with Intellectual Disabilities and Developmental Disabilities) is for individuals age 5 and over who have intellectual disabilities or other developmental disabilities.  It is designed to assist Medicaid beneficiaries to live in the community and avoid institutionalization.  View the Funding Sources & Services flow chart.

Choosing or Changing Service Providers:
                                                                                   

If you are choosing a provider (for TCM, Day, or Residential) for the first time or are interested in changing
existing providers you may do so at any time.  In order to begin this process please contact the CDDO
Liaison Pennie Dubisar-Cross at (785) 842-0550. 

The CDDO Liaison is responsible for initiating the paperwork for all service provider choices (either initial
choices or changes in provider).  The Liaison will also be able to provide you with information on how to select
the appropriate service provider as well as the availability status of all service providers in our CDDO area.
